Originally released in 2006, the band's third studio album follows The Patient as he confronts death and revisits his life. Produced with Rob Cavallo, it moves between marching rhythms, piano, distorted guitars, and dramatic vocals. This 20th anniversary deluxe edition expands the album.

"Welcome To The Black Parade" starts with a solitary piano note and grows into a marching-band-inspired rock anthem, while "Teenagers" uses a stomping beat and a sarcastic look at youth culture. "I Don't Love You" shifts into a slower breakup song with a chorus, and "Mama" incorporates theatrical exchanges and a guest performance by Liza Minnelli. The differences in pacing and arrangement let the band move from private regrets to public spectacle, changing intensity within individual songs and choruses.

The Patient's story deals with mortality, fear, memory, and loss. For this 20th anniversary deluxe edition, six bonus recordings join the original album: three rare B-sides and three performances from the band's 2007 tour. The live selections include "Someone Out There Loves You," a song long known to fans as "Stay" that was never recorded in a studio. James Jean created new anniversary artwork.
